RECAP:

1. This meeting will be on zoom.
2. Your zoom link can be found in your email.
3. For Doubles–make sure the other person takes their Neurotoxicity Assessment!
4. If you need to reschedule use link sent to your email! (We are very strict on rescheduling for this appointment because it is usually a $97 call and we are BOOKED–1 free reschedule outside of 24 hours)
5. If you need to reschedule within 24 hours reach out to your Point of Contact and they can reschedule you for a $37 reinstatement fee (this time slot is 100% dedicated to you.)  
6. If you’re married (or have someone else that’s a part of your support system) they need to be there as well!

What You Should Come Ready With:

1. Neurotoxicity Assessment Completed
2. Know Where to Find Your Zoom Link!
3. Be in a Quiet Place Ready to Dive in to What’s Been Going On

Cancellation/Rescheduling Policy:

Please reach out to your point of contact (they texted you about your booking) to reschedule.

This time slot has been personally dedicated to you and cannot be filled with another appointment within 6 days. First reschedule before 4 days is free. Second reschedule is $37.

If rescheduled within 4 days there is a $37 reinstatement fee.

If no notice is given 12 hours prior to meeting time and meeting is “no-show’ed” there is no refund for your appointment.
